Inspector’s narrative

What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. LPA took a census at the start of this visit. The UIR was received by the licensing agency on 2/25/2026. It indicates that on 2/20/2026 1 child in care exited their room and entered the outside playground where they remained for about 2 minutes. During this time the child was on the play ground without any staff supervision or visual observation, while all staff were inside. Staff were informed that the child was on the playground by themselves and staff brought the child back inside and resumed visual observation of the child. No injuries were reported. At the time, all other children and staff were inside preparing to go outside to the playground, with the door to the play ground propped open. - Facility records were reviewed and pertinent parties were interviewed. Based on the information gathered, the following violations have been identified: 1 child in care was left without visual observation for about 2 minutes. SEE LIC809D for deficiency cited. - LPA Chase Atherton informed Director Linda Mogk that this report dated 3/3/2026 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. - Also, LPA Chase Atherton informed the Director Linda Mogk to provide a copy of this licensing report dated 3/3/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. - SEE LIC809C for a continuation of this report... A notice of site visit was given to Director Linda Mogk and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to the Director Linda Mogk.
