What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 10/16/2025. It indicates that a Child (Child 1 [C1]) was bumped on the hand by a staff member (Staff 1 [S1]) using a tissue box, 3 times to get C1's attention. Facility records were reviewed and pertinent parties were interviewed. Based on the information gathered, the facility acted appropriately, and no violations have been identified at this time. The facility conducted their own investigation into the matter and provided S1 more appropriate ways to get a child's attention. C1 did not seem to react to this incident at all, as if nothing had happened. There is also conflicting information about what exactly happened during the incident. LPA provided a LIC9102TA Advisory Note which detailed best practices with regards to children's personal rights. This is not a notice of deficiency. An exit interview was conducted and a copy of this report was provided to facility representative Liz Martinez. A notice of site visit was given to facility representative Liz Martinez and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
