What the inspector wrote

A case management inspection is being conducted in response to the receipt of an Unusual Incident Report (UIR) from the facility. Licensing Program Analyst (LPA) Blanca Ruiz met with facility director, Anna Cruz t o follow-up on the UIR submitted to the department. Parent(s)/Legal guardian of child(ren) of the involve in the incident are aware of all details. Facility serves age group children from birth to 24 months. Facility was toured and census was taken. Report indicates that during free play Child#1 was standing by the music area when Child#2 ran across the classroom and pushed Child#1 causing child to fell down sideways landing on the wooden cubby located in the corner of the classroom by the back wall. Child#1 hit the left side of the face, a bump was observed on left eyelid below eyebrow area. Child#1's parent(s)/Legal guardian(s) were contacted immediately and child was taken to Urgent Care. Child#1 was taken to the doctor and stiches were needed to close the injury. Per records review, child had a cut approximately \302\275 inch on the outer corner above the eyelid. Child#1 returned to the facility the following day. Interviews were conducted with pertinent parties present at the facility during the incident and with parent(s)/legal guardian(s) of Child#1. Records reviewed indicate that child retuned to daycare the following day and parent(s) legal guardian (s) confirm the information. Child#1 was cleared by doctor and no other issue follow before or after the incident in question. Child is no longer in care as family relocated to another daycare closer to home. The day of the incident 6 infants were present in Classroom C4B with 3 Staff ( Staff#1- Staff#3). Teacher to Child ratio 1:4 infants. During inspection, LPA inspected infant(s) room(s) and found them in substantial compliance. Per Director, Classroom C4B has been rearranged and the wooden cubby has been relocated to an area away form the playground area where children play. Staff had a discussion to reassess room to prevent other incident(s) occurrences. Based on information gathered, the facility acted appropriately, and no violations have been identified. An exit interview conducted, and report was reviewed with licensee, Anna Cruz. Appeal rights were discussed, and A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC, UPON THEIR REQUEST, FOR THREE YEARS.
