What the inspector wrote

On 12/13/2024 at 3:30 PM, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a Case Management inspection. The Case Management inspection was conducted in response to the receipt of an Unusual Incident Report (UIR) from Licensee Naresh Bakshi on 12/09/2024. It was reported that the incident took place on 12/05/2024. Further details are as follows. On 12/06/2024, The Riverside Regional Childcare Office (RRO) received a telephone call from Licensee Naresh Bakshi, reporting an unusual incident. The licensee stated that on 12/05/2024, a child became partially unconscious while playing with a bicycle underneath the play structure. Licensee Naresh Bakshi stated that the child\342\200\231s neck was stuck between the play structure and the bicycle handle. Licensee Naresh Bakshi stated that he contacted 911 immediately. It was stated that a staff member at the facility administered CPR on the child. The child was transported to a nearby hospital. Licensee Naresh Bakshi submitted a licensing form (LIC 624) Unusual Incident Report to the Department on 12/09/2024. The child has since been released from the hospital. During today's visit, LPA made observations, obtained pertinent documentation, discussed details with licensee, took a tour of the facility, and took a census of all children present. LPA informed Licensee that due to the severe nature of the reported incident, IB investigator Philippe Ryan Miles will arrive at the facility on a later date, in order to conduct a follow up visit. LPA conducted an exit interview with Licensee Naresh Bakshi, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
