Inspector’s narrative

What the inspector wrote

Interviews with multiple individuals confirmed that some children were engaged in an inappropriate game under a table. It was reported that staff did not witness the behavior because objects had been placed around the table, obstructing their view. The game reportedly continued for several minutes without staff awareness. During the activity, some children expressed feeling uncomfortable. Based on the interviews conducted, it has been determined due to lack of supervision, daycare children engaged in inappropriate behavior. Therefore, the allegation is substantiated, meaning the preponderance of evidence has been met. See LIC 9099 D page for citation. LPA informed the director that Type A citation must be reported to all authorized representatives/guardians of all children currently enrolled by the next business day, or the next day children are in care, and all newly enrolled children for the next 12 months from the date of citation. The signed Acknowledgement of Receipt LIC 9224 must be placed in child’s file for verification. An exit interview was conducted with director. During the exit interview, appeal rights were discussed/provided, Notice of Site form provided, LIC 9224 Acknowledgment of Receipt form provided, and a copy of this report was provided. Notice of Site visit must be posted for 30 days.
