What the inspector wrote

On 02/25/2026 at 2:15 PM, Licensing Program Analyst (LPA) Tiffanie Diep met with Director Mireya Gomez for the purpose of an unannounced case management inspection to follow up on an Unusual Incident Report (UIR) received by the Department on 02/10/2026. LPA observed two staff supervising 15 children. All individuals subject to a criminal record review have obtained a criminal record clearance. Since the previous visit, LPA obtained supporting documentation pertinent to the incident and conducted interviews with pertinent individuals involved in the incident. It was alleged two children (C1 and C2) engaged in inappropriate behavior while being transported to the facility. Interviews conducted disclosed the staff member (S1) present at the time of the incident did not witness the incident as they were driving the vehicle. Conflicting information was obtained regarding what took place between C1 and C2. Information obtained revealed staff have separated C1 and C2 during pick-up once they were made aware of the incident. During today’s visit, LPA observed staff maintain visual observation of children in care ensuring adequate supervision at the facility. It is determined there was a lack of clear corroborating evidence to prove the alleged incident did or did not occur. Based on observations made at the facility, information obtained during interviews, and records reviewed, it is determined there were no violations pertaining to the incident. There were no deficiencies cited at this time. An exit interview was conducted and report was reviewed with the director, Mireya Gomez. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
