What the inspector wrote
On Friday October 3, 2025, at 10:33 AM, Licensing Program Analyst (LPA) Giovanni Cristales met with Assistant Valeria Rodin Cisneros, who guided analyst on a facility tour. The purpose of the inspection is to conduct an unannounced Annual Inspection. Licensee is licensed to provide care and supervision for a Large Family Child Care for a capacity of up to14 children. LPA toured the areas of the home utilized for the Family Child Care to ensure the home follows Community Care Licensing Title 22 Regulations. Present during the time of this inspection is Licensee and two (2) adults (Assistants), one (1) infant child, and five (5) toddler children. Family members residing in the home include four (4) adults (Licensee, licensee's husband, adult daughter, son-in-law, and two minor children). All adults residing in the home have a Criminal Record Clearance. Days/hours of operation are Monday through Friday, 6:00am \342\200\223 6:00pm. Incidental Medical Services (IMS) policy was discussed.
Physical Plant:
This is a single story, 5-bedroom, 3-bathrooms home with kitchen/dining, formal dining room, living room, laundry room, playroom, enclosed patio, laundry room, garage and backyard. There is no pool/spa or body of water on the premises. LPA did not see any pool/spa or bodies of water on the premises.
Main care is provided in Bedroom #5 at end of hallway upon entrance to the room, and enclosed is the playroom on the left. Children use the bathroom in hallway on the right. Children have access to formal living rooms, kitchen/Dining area, and back yard. The home was inspected inside and out for safety, comfort, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents/cleaning compounds
(made inaccessible to children, stored in the laundry)
, medicines
(made inaccessible inside locked pantry),
and hazardous items
(sharp knives in upper cabinet above the kitchen sink with safety latch)
that can pose a danger to children.
Fire/earthquake drills complete and maintained current (08/05/2025). Roster complete and maintained current. The smoke detector and carbon monoxide detector are in operable condition. PA observed the required fire extinguisher (2A10BC) is fully charged. There are no weapons/firearms in the home. The facility sketch is complete and current posted in hallway to the left; there is working telephone (land line and cell).
Off limit areas:
Include Bedroom #1 - #4, bathroom #1 and #3, laundry room, enclosed patio, garage
(all made inaccessible with key locks).
Kitchen:
Home has a clean and fully stocked refrigerator/freezer.
No chemicals in the kitchen were observed to be accessible. Cleaning supplies are in both (off limits laundry room, and garage made inaccessible with key locks). Children bring own lunch and snacks.
Bathroom
: Bathtub/shower are free of hazards. The following are inaccessible: Sharp items, mouthwash, shampoo, razor, nail polish. Toilet and faucet are clean and operable. LPA observed one infant changing table, medicine cabinet with plastic latch. No hazardous items were stored underneath the sink cabinet.
Outdoor:
The backyard is completely fenced all around (Vinyl). Yard is composed of concrete and turf flooring for safe playing. All playing areas have turf underneath to soften any falls. There is cushioning or resilient material, new 4-inch rubber flooring around the play structures. There is a jungle gym that is anchored, Little Tikes toys, padded/gated infant area on the patio, swing that is anchored, garden (fenced), kennel in the rear and gazebo. LPA observed four (4) dogs, two dogs in each (kennel). Per assistant, the dogs do not interact with children in care. Per assistant all the shot records are up to date. The play area is clear and clean of debris, play area is fenced and gated all around, no body of water on the premises. There is a swing/slide that is anchored and other age-appropriate toys observed to be free of hazards. There are two separate shaded areas. LPA Observed Air condition units inaccessible with fencing all around and protective screening over the unit. Per assistant there one cat (off limits master bedroom). LPA did not observe the cat during the inspection. There is a fenced corridor only for the dogs which is next to the playing area. Dogs only use the corridor after business hours.
Advisory/Other:
First Aid kit was observed with supplies readily available. CPR/First Aid 12/07/2026, Mandated reporter Training (12/17/2026). Electrical outlets
(are inaccessible with plastic cover),
no baby bouncers saucer chairs, or any recalled and prohibited toys or sleep/ play equipment were observed on the premises. Window cords throughout the home are inaccessible.
Documents Provided and or Discussed: The following were observed to be in regulation of Title 22 requirements: Fire Drill Log is current, Roster is up to date, Postings were observed and meet Title 22 requirements, Safe Sleep and Lead Poisoning flyer's observed, Staffing and Ratio in regulation large capacity, pet in the childcare setting.

LPA discussed the safe sleep regulations with the licensee and also instructed the Licensee to visit the Child Care Licensing Safe Sleep webpage at

as an additional resource.
LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following was discussed with the Licensee:
Mandatory licensing forms for the children\342\200\231s files, facility forms/records, and information to be posted in the family child care home; Requirements to conduct fire and disaster drills once every six months and record it; Role and responsibilities of being a mandated reporter (

) were reviewed;
Licensee were reminded that 100% supervision is required at all times to children in care; Licensee were made aware that it is her/their responsibility to know the regulations as well as anyone who assists in providing care; Licensing must have the facility\342\200\231s phone number at all times; if the phone number is changed, licensing must be notified; Regulation prohibits the smoking of any kind during the operation of the day care.
Licensee was advised of the requirement to report Unusual Incidents. A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above. Licensee was informed to utilize the

Licensee advised of the Notice of Site Visit, which must be posted at the entrance of the facility for a period of 30 days whenever a licensing inspection is conducted. If a Type A deficiency is cited, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224. If these requirements are not met, civil penalties per violation will be assessed.
Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, inspect the facility. The Licensee shall permit the Department to inspect the family childcare home and to privately interview children or staff, to determine compliance with or to prevent violations of family childcare laws or regulations, also enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation.
Prior to making alterations or additions to a family childcare home or grounds, the licensee shall notify the Department of the proposed changes, including, but not limited to, the following: Conversion of a garage (either attached or detached) into a "childcare" room; Room additions to the family childcare home.
Any change from an area of the family child care home previously identified as "off limits" to an area where care and supervision will be provided to children in care. The licensee shall provide the Department with a copy of an inspection report when an inspection is required by the local building inspector as a result of the alteration, addition or construction.
Notice of Site Visit: A notice of site visit was given and must remain posted for 30 days.
