What the inspector wrote

On Tuesday, August 19, 2025, Licensing Program Analyst (LPA) Braddock met with Lead teacher Monserrat Romo who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management Incident inspection regarding the UIR that was reported to Palmdale Regional Office on 8/7/25. Description of incident: while C#1 was playing with toys in the classroom, C#2 who was holding different toys, approached C#1. C#1 got upset and scratched C#2 thereby causing injury to C#2\342\200\231s inner ear and jaw/cheek area. C#2 was cleaned with wet wipes and was applied with antibiotic ointment to avoid infection. C#2 was not taken to the Urgent Care. Parents of C#1 and C#2 were contacted at around 11:35 A.M. At around 11:45 A.M, C#1 tried to do the same thing to C#3, but C#1 was only able to tag the neck of the shirt of C#3. LPA Braddock conducted interviews with staff and children involved. No further investigation is needed. No deficiencies have been cited at this time. An exit interview was conducted and a copy of this report was provided to the lead teacher , along with a Notice of Site Visit and her appeal rights.
