What the inspector wrote

On this date and time, Licensing Program Analysts (LPAs) Taityana Benson and Laura Mejorado arrived at the facility to conclude the investigation on separate manner, a previous inspection was conducted on September 05, 2023. LPA met with Alisa Holtegaard, Interim Director and conducted a tour of the facility inside and outside. During the previous visit, on September 05, 2023, at 9:35 a.m., LPA observed an overall total of 26 infants amongst Infant A, Infant B, and Toddlers 1 classroom. The Director confirmed all infants present are enrolled. The Director confirmed in Infant A, Infant B and Toddlers 1 classroom ages are under 2 years old. LPA asked the Director why 26 infants are in care. Director stated due to the holiday the parents of part-time enrollees were thrown off their schedule. Director stated some infants were not scheduled to attend that day, but the center allowed them to be dropped off and will speak to the parents about upholding their enrollment agreement. LPA informed the director that at no time shall the facility provide care to more than 24 infants. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. SEE LIC 9099-D for Type A deficiency cited An exit interview was conducted and report was reviewed with Alisa Holtegaard, Interim Director. LPA Taityana Benson informed Alisa Holtegaard, Interim Director that this report dated 12/06/23 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA Taityana Benson informed Alisa Holtegaard, Interim Director to provide a copy of this licensing report dated 12/06/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled chindren for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

  • STAFF-INFANT RATIO

    101416.5(b) Staff-Infant Ratio, there shall be a ratio of one teacher for every four infants in attendance.This requirement is not met as evidence by: Based on interview and record review, the Licensee did not comply with the section cited above, the staff-infant ratio was not within 1:4 (one staff to 4 infants) the week of August 28, 2023, which poses an immediate Health and Safety, Personal Rights risks to persons in care.

    LIMITATIONS ON CAPACITY

    101161(a) Limitations on Capacity, a licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation.This requirement is not met as evidence by: Based on LPA observation, the Licensee did not comply with the section cited above on 09/05/2023, LPA observed a total of 26 infants in the infant classrooms and the maximum capacity is 24 infants, which poses an immediate Health and Safety, Personal Rights risks to persons in care.
