What the inspector wrote

On the above noted date and time, Licensing Program Analyst (LPA's), Diana Brasel and Griselda Castellon conducted an unannounced Case Management inspection for an increase of capacity. Upon arrival, LPA met with Asst. Director Sarah Ellmore. During visit director Alisa Holtegaard arrived and continued the visit with LPA's. Licensee is requesting to increase their current infant capacity from 24 to 32 children ages 6 weeks to 2 years of age in rooms Infant 1, Infant 2, Toddler 1, and Toddler 2. Hours of operation will be Monday - Friday 6:30 am - 6:30pm. Present during today’s inspection were ( 11 ) children with (4 ) staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. LPA continued to tour the facility and measured new additional indoor and outdoor activity space. Total indoor activity space measured 1646.47 , which is sufficient to accommodate the requested capacity of (32) c hildren. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filtered drinking fountains, children's own water bottles with filling with facilities filtered water. There are a total of 16 cribs, 2 potty chairs and 1 toilet. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 04/21/2025. LPA observed a total of (3) sinks and (3) toilets available for children’s use. These are sufficient to accommodate the requested capacity of (32) children. The facility currently has a fully fenced playground area. Fencing is a block wall. chain link, and wrought iron, and is at least four feet high. The total square footage for all the outdoor activity space is 3040.00 , which is sufficient to accommodate the requested capacity. Shade is provided via large permanent canopy structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure which is age appropriate on the playground for children, which is properly anchored. There is adequate cushioning in fall zones of climber provided by artificial turf and rubber matting. Drinking water is available via fountain and children's water bottle. LPA observed all hazardous items on the playground to be inaccessible to children. Director Alisa Holtegaard was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. High chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. High chairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. Crib area is separated from activity space via plexi glass. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in infant rooms. Needs and Services Plans and Sleep Plans are kept in infant rooms. There is at least one potty chair or toilet for every five infants along with 3 changing tables. LPA observed a total of (3) sinks available for children’s use. These are sufficient to accommodate the requested capacity of (32) children. There is a separate staff restroom equipped with a toilet and a sink. LPA's reviewed with director the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted. No corrections are needed. An increase in capacity from (24) children to a total of (32) children will be granted upon a final file review. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with Alisa Holtegaard Director.
