What the inspector wrote

Upon receipt of a Type A deficiency the licensee shall post the report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report to parents/guardians of children in care. This report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & the licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from the parent/guardian & place it in each child's file. If these requirements are not met, civil penalties will be assessed. An exit interview was conducted, and a copy of this report was provided to the Director Katrina Henderson along with a Notice of Site Visit and Appeal Rights.

This requirement was not met as evidenced by:Based on observation, interviews, and record review, it was revealed that the staff #1 handled the child in a rough manner while in care which poses an immediate risk to the health and safety of children in care.
