Inspector’s narrative

What the inspector wrote

On 07/16/2025, Licensing Program Analysts(LPAs) Eric Ramos and Laura Mejorado arrived at the facility to conduct a Case Management visit. LPA met with Licensee Nicole Ingram, toured the facility, took census, and discussed the following: On a previous Case Management report conducted on 06/20/2025, LPAs Eric Ramos and Elyse Jones discussed with assistant Dominique Patterson of an altercation that took place between the licensee and another individual at the facility during operating hours on 05/01/2025. It was determined that although unknown individuals were present during the altercation, there were no daycare children present. This altercation resulted in a violation of Title 22 Regulations which was discussed in the report dated 06/20/2025; however, no citation was given. On today\342\200\231s visit, the Licensee has been cited for the altercation for Conduct Inimical, Section 102402(a)(3) which states the Licensee shall "conduct in the operation or maintenance of a family day care home which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of the State of California." See LIC809-D for cited deficiency. LPAs Ramos and Mejorado also informed Licensee to provide a copy of this licensing report dated 07/16/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Additionally, LPAs Eric Ramos and Elyse Jones conducted a required/annual inspection on 06/20/2025 where the Licensee was cited several deficiencies that needed corrections that were due on 06/21/2025 and 07/04/2025; however, the plan of corrections have not been received by LPA. During today\342\200\231s inspection, LPA Ramos provided Licensee with a letter stating the plan of corrections given on 06/20/2025 needs to be submitted to Community Care Licensing on or before Friday, July 18, 2025. Failure to provide proof of corrections on or by 07/18/2025 will result in assessment of civil penalties. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee Nicole Ingram.
