What the inspector wrote

On 10/23/2025 at 9:35 AM Licensing Program Analyst (LPA) Claudia Caywood arrived at the facility to conduct a 3-year required inspection. LPA was granted entry by Director, Michelle Martinez. LPA toured the facility, inside and out, reviewed records, and observed and/or discussed the following: This is an Infant, Preschool, and School Age Center. A review of the staff records and a review of children's records were conducted as part of this evaluation. The inspection consisted of reviews of the following domains: \342\200\242 Food Service \342\200\242 Reporting Requirements \342\200\242 Physical Plant \342\200\242 Care and Supervision \342\200\242 Children Records \342\200\242 Staff Records \342\200\242 Staffing Ratio and Capacity \342\200\242 Personal Rights The inspection found the facility to comply in these domains. The inspection consisted of reviews of the CARE tool domains. The inspection found the facility to comply except as noted on the LIC809D. Deficiencies were cited on this visit. (CONT LIC 809-C) The licensee has been asked to update the following documents, if applicable, and submit to licensing within 30 days: 1. LIC 500 Personnel Report 2. LIC 610 Emergency & Disaster Plan 3.Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) 4. LIC 309 Administrative Organization 5. LIC 308 Designation of Administrative Responsibility The following items have been posted and are updated where necessary: \302\267 License, Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) \302\267 Parent\342\200\231s Rights Poster (PUB393), Personal Rights (LIC613A); Child Car Seat Law, Menu \302\267 The facility is operating within the terms of the license \302\267 Appropriate supervision was provided during this inspection \302\267 Classrooms are equipped with age appropriate furniture and equipment in good condition \302\267 Classrooms are clean and free of hazards \302\267 No weapons stored at the facility \302\267 There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Medications are stored where inaccessible to children \302\267 Hazards are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous \302\267 Poisons and toxins are locked \302\267 All floors are clean and safe \302\267 Bathrooms were observed to be safe, sanitary and in operating condition \302\267 Playgrounds are enclosed by appropriate fences and free of hazards \302\267 Outdoor activity areas are supplied with age and size appropriate equipment in good condition (CONT. LIC 809-C) \342\200\242 All storage containers for solid waste, including moveable bins-have tight-fitting covers and in good repair \342\200\242 Uncontaminated drinking water readily available both indoors and out- water pitcher and cups. \342\200\242 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall: natural grass \342\200\242 Sign in/Sign out record was reviewed and does meet regulation requirements \342\200\242 A Staff member is present with current Pediatric CPR/First Aid which expires on 2/2027 \342\200\242 Opening and closing staff member\342\200\231s CPR/First Aid expires on 2/2027 (Opener) & Closer) 12/2026 \342\200\242 Director completed Health and Safety Training on 11/6/2024 \342\200\242 Staff qualifications were reviewed \342\200\223 health screening is on file and all staff meet educational requirements and health requirements for performing assigned tasks \342\200\242 Staff have received on the job training for house keeping, sanitation and universal health precautions \342\200\242 Required records for children contain Identification and Emergency Information Licensee was informed of the Department has inspection authority per Health and Safety Codes sections: 1596.852, 1596.853 and 1596.8535. \342\200\242 Documentation of fire & earthquake drills to be conducted every six months: Last drill on 10/16/25 Licensee was informed of UnusualIncidentReportingemail: UnusualIncidentReportsDO09@DSS.CA.Gov Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. L PA discussed the safe sleep regulations with licensee or facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee or facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. (CONT. LIC 809-C) This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS informatio n see PIN 22-02-CCP . Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test th eir water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. However, a lead test report was not available to be reviewed by the LPA on the day of annual inspection. (CONT. LIC 809-C) LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Licensee or facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
