A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 9/5/2025. It indicates that in the morning, when children were exiting a classroom to move to the outside playground area, Child 1 (C1) was left in the classroom alone at 8:08 AM. C1 was left alone in the classroom for a total of about 6 minutes. Then, a parent of another child saw C1 through the window of the classroom and immediately notified Staff 2 (S2) that C1 was alone in the classroom. At 8:14 AM, S2 immediately entered the classroom and attended to C1 and brought them outside where the rest of C1's class was playing. Facility records were reviewed, including video footage, and pertinent parties were interviewed. Based on the information gathered, the following violations have been identified: C1 was left without any visual observation from any staff for 6 minutes which is an absence of supervision. California Code of Regulations Section 101229(a)(1) has not been followed and is being cited on the separate LIC809D. A civil penalty has been assessed during this inspection because of the absence of supervision of C1, in the amount of $500. See LIC809D and LIC421IM for more details. LPA Atherton informed facility representative Kellee Nelson that this report dated 9/12/2025 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Atherton informed the facility representative Kellee Nelson to provide a copy of this licensing report dated 9/12/2025 that documents any Type A citation(s) to parents of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to facility representative Kellee Nelson and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
