What the inspector wrote

On January 21, 2026, Licensing Program Analyst (LPA), Calloway conducted an unannounced inspection to the above facility. The purpose was to conduct a case management inspection. LPA met with Shelley Salas, Facility Representative who granted access. LPA toured the facility inside/outside with the representative and observed seventeen active day care children aged 3- 5 years old and three staff in care. LPA and the representative toured the outside area. The children are able to reach the outdoor activity area safely from the preschool classroom. LPA and representative observed a large play structure as play equipment with a large slide, two climbing areas, and steps with the height appropriate for the age group of children. There are wood chips as a cushioned surface. The playground is enclosed by a chain-linked fence to protect children and to keep them in the outdoor activity area. The fence measured sixty-six inches high and is secure around the entire playground. Representative stated the play structure and play ground will be shared with four to five TK classes that are across the playground and in the same area of the preschool. LPA observed a sign in front of the play structure that states “This play structure is designed for kids aged 5- 12” years old. Therefore, further review and documentation for the manufacturing and/or modification of the play structure is required. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit (LIC 9213), were given to Shelley Salas, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
