On 9/26/24 at 8:20 am, Licensing Program Analyst (LPA) Patricia Berry conducted a case management visit to deliver an amended report from 8/28/24. LPA met with assistant director was granted access into the facility, toured facility and took a census. On 9/26/24 LPA amended the 8/8/24 report to reflect a change in the violation from "admission agreement" to "personal rights". Director agreed to provide the amended C and D pages of the report to all children currently enrolled by the next business day, or the next day children are in care, and all newly enrolled children for the next 12 months from the date of citation and must be placed in child’s file for verification. Exit interview conducted with assistant director, report, appeal rights, and notice of site visit issued. Notice of site visit must be posted for 30 days.
