What the inspector wrote

On May 22, 2025, Licensing Program Analysts (LPAs) Elyse Jones and Chase Atherton arrived at the facility to conduct a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) from the facility. The LPAs toured the facility and took census. The UIR was received by the licensing agency on May 16, 2024. It indicates a parent at the school accidentally crashed through the school causing damage to classroom #6. At the time of the accident there were no children or staff in the room and no injuries were reported. Additionally, the UIR stated a waiver request would be submitted to the Department to request moving the children who are assigned to classroom #6 to a different licensed facility. During a follow up call with the Licensee she stated the children were already attending the other licensed facility and all of the repairs, except the window, were completed over the weekend. The LPA informed the Licensee a waiver request must be reviewed by Management and granted prior to making any changes to the daily operation that could be a violation of the Title 22 regulations, in this case that did not happen. However, it was determined the children were moved to an appropriate classroom in the other licensed facility, the fire clearance was not violated, and all Authorized Representatives were notified, therefore, a citation will not be issued during this inspection. On May 21, 2025 the children returned back to their original classroom and no other incidents were reported. All necessary documentation regarding the incident and repairs have been collected. Based on information gathered, the facility acted appropriately, and no violations have been identified. The Licensee immediately ensured all children and staff were safe, Law Enforcement was contacted, Licensing was contacted and parents were notified their children would be temporarily attending a different licensed facility until repairs were completed. The facility is planning to install bollard posts in front of each parking spot that faces the facility in efforts to prevent a future accident. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Toni Espino, Director.
