What the inspector wrote

On the date and time listed above, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to conduct a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) from the facility. LPA toured the facility and a took census. The UIR was received by the licensing agency on 2/18/2026. It indicates an authorized representative of a child enrolled at the school, accidentally crashed their car into the school building causing damage to the wall of classroom #2. At the time of the accident there were 13 children in the room and 2 minor injuries were reported such as a small cut or a small bruise. Children were immediately removed from the room and provided first aid if necessary. All authorized representatives were notified of the incident. Additionally, the report of the UIR stated the facility would have all the children that normally attend room #2 temporarily transferred to the licensee's other site across the street, facility number 364813832, until repairs in room #2 are completed. The facility stated they had all authorized representatives of the children sign an agreement stating they understand their child would be temporarily transferred to the other site. The facility also has installed 7 additional metal bollards to prevent any future cars from crashing into and damaging the facility. The facility also has 3 additional metal bollards that have yet to be installed. Records were reviewed, and pertinent parties were interviewed. Based on information gathered, the facility acted appropriately, and no violations have been identified. The facility notified licensing per Title 22 regulations. The room the facility used to care for the displaced children has an appropriate fire clearance per Title 22 regulations. Based on the information gathered the facility has taken the appropriate steps to repair the damage, treat minor injuries, and provide another location for children to receive care and supervision. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Facility Representative Ranjanie Serasinghe.
