Non-compliance follow-up·January 8, 2026

On Thursday, January 8, 2026, an unannounced Legal Case Management inspection was conducted by Licensing Program Analyst (LPA) Giovanni Cristales. LPA met with Licensee Brenda Guzman and advised her the purpose of the visit. Licensee provided LPA with a tour of the facility. LPA observed seven children in care at the time of the visit. During the inspection, the LPA explained the purpose of the visit and provided the licensee with a copy of Decision and Order \342\200\223 CDSS No. 7925104002-B, which prohibits Monique Crowfield from employment and excludes her from all State-licensed facilities. The Decision and Order became effective December 29, 2025. The LPA toured the facility and did not observe Monique Crowfield on site. The licensee stated that she understands the terms of the Decision and Order and had no questions. No deficiencies were identified during today\342\200\231s inspection. An exit interview was conducted with the licensee. A Notice of Site Visit (LIC 9213) was posted and must remain posted for 30 consecutive days. Failure to maintain the posting as required may result in a $100 civil penalty. A copy of Appeal Rights (LIC 9058 FAS 01/16) was provided to the licensee.
