What the inspector wrote

On May 21, 2026, Licensing Program Analyst (LPA) Hanna Cha met with Site Supervisor Gina Nino. The purpose of the visit was to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the Site Supervisor and toured the facility. LPA observed two teachers providing care and supervision to 16 preschool-age children. The current days and hours of operation are Mondays through Fridays; 6:30 am to 05:30 pm. Physical Plant/Indoor/Children’s Area: The facility operates out of four classrooms, two children's bathrooms, one staff bathroom, one kitchen, an outdoor play area, and a detached staff office. The classrooms were observed for age-appropriate toys/equipment, safety, and good repair. LPA observed fire alarm and carbon monoxide alarms located throughout the facility. LPA observed cots and individual bedding for children. The kitchen was observed for safety, sanitation, and good repair. The facility provides breakfast, lunch, and two snacks. Drinking water provided via water pitchers and plastic cups. LPA observed two children’s bathrooms for safe and sanitary operating conditions. Toilet paper, paper towels, and soap available to children. LPA observed the outdoor play area for age-appropriate toys/equipment, safety, cushioned flooring, good repair, and shady areas. Per LPA observation, there are no pools/spas/bodies of water at the facility. File Review: LPA reviewed two children files which included all required documentation. LPA reviewed two staff files. Per record review and interview, fingerprint clearance for Staff #1 (S1) not cleared and associated to the facility. S1 was hired on 10/17/2025 and was present at the facility since hire date. The facility submitted a criminal record transfer to the department on 10/16/2025. The department sent a letter to S1 on 01/08/2026 informing S1 that fingerprints were not received. At time of inspection, S1 was present at facility. LPA informed Site Supervisor that the fingerprints for S1 were not cleared. LPA informed that S1 may not return to the facility until fingerprint clearance is received. LPA provided a Live Scan form and informed Site Supervisor and S1 to receive clearance for DOJ, FBI, and CACI. S1 immediately left facility to re-submit fingerprints. A Type A citation issued. See 809-D. A civil penalty of $500 assessed ($100/day, up to maximum five days). Last lead testing conducted on 08/2023. Last disaster drill conducted 05/2026. Other: Per Site Supervisor, smoking is prohibited on the premises. Per Site Supervisor, there are no firearms/weapons stored on the premises. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ) Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
