Inspector’s narrative

What the inspector wrote

On the time and date listed above, Licensing Program Analyst (LPA) Raymond Moorehead, arrived at the facility for the purpose of conducting an inspection on a separate matter. LPA was greeted by Licensee Lalita Bakshi and granted entry to tour the facility. LPA conducted a census of preschool 13 children in care, with 1 teacher supervising. During the course of the inspection regarding a separate matter, LPA observed the preschool classroom become out of ratio. Upon LPA's arrival, LPA conducted a census of a total of 11 children present. LPA then observed a total of 2 more children arriving in the preschool classroom, making the overall ratio 13. At the time of arrival of the 13th child, the Licensee had stepped into the infant room to provide breaks for the infant teachers. It was noted that a staff member did not show up to work today, with a last-minute call off, according to the Licensee. Licensee also stated that the 13th child was not expected to arrive for care today. Therefore, based on LPA observations, it was determined that the facility was temporarily operating out of ratio for approximately 35 minutes. See LIC 809-D for cited deficiency. No civil penalties were issued. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. LPA informed Licensee that this report dated 10/21/2025 documents 1 Type A citation, which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, and personal rights of children in care. Also, LPA informed the facility representative to provide a copy of this licensing report dated 10/21/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted and report was reviewed with Licensee Lalita Bakshi.
