What the inspector wrote

On 8/9/23 at 11:50 am, Licensing Program Analyst Patricia Berry conducted a case management visit conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 8/7/23. Facility records were obtained. LPA interviewed staff. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided to the licensee. An exit interview was conducted with director, and a copy of this report was provided, appeal right and notice of site visit. Notice of site visit must be posted for 30 days.
