What the inspector wrote

Exit interview conducted with facility representative, Alicia Flores and a copy of this report, appeal rights and notice of site visit were left with her. Based on LPAs observations, interviews conducted, and record reviews, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. California Code of Regulations (Title 22 regulation - see LIC9099D) requires licensee to ensure staff treat children in care with dignity and practice safe sleep regulations. See PIN-20-24-CCP for safe sleep requirements. 1 Type B deficiency and 1 Type A deficiency will be assessed for the above violations. A copy of the licensing report must be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. If these requirements are not met civil penalties per violation will be assessed.
