On the date and time listed, Licensing Program Analyst (LPA) Taityana Benson conducted an annual inspection as part of a compliance review. A tour of the inside and outside of the facility was granted and the following was observed and/or noted: This is a combination center and the other licensed program(s) are daycare/school age, which school age was also inspected on this date. A review of staff and children's records were conducted as part of this evaluation. The director is asked to update the following documents, if applicable, and submit to licensing within 30 days: - LIC 500 Personnel Report - LIC 610 Emergency & Disaster Plan - Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made) - LIC 309 Administrative Organization (only if changes have been made) - LIC 308 Designation of Administrative Responsibility (only if changes have been made) The following items have been posted and are updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent\342\200\231s Rights Poster (PUB393) -Personal Rights (LIC613A) - Child Car Seat Law - Menu \302\267 The facility is operating with the limits as stated on the license. \302\267 Ratios are being met during this inspection. \302\267 Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards. \302\267 There are no weapons present at the facility as stated by Licensee Tammy Murphy. \302\267 There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. \302\267 Medications are not present as stated by Licensee Tammy Murphy. \302\267 Hazards are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous. \302\267 Poisons and toxins are locked. \302\267 All floors shall be clean and safe. \302\267 Sanitation solution is out of the reach of infants. \302\267 Highchairs and tables have broad based legs and trays lock into place (See LIC809-D). \302\267 Changing tables are within arm\342\200\231s reach of a sink; padding is at least 1\342\200\235 think with raised sides of at least 3\342\200\235 covered in washable vinyl or plastic. \302\267 Hand washing is completed before and after each diaper change and before feeding an infant. \302\267 Bathrooms were observed to be safe, sanitary and in operating condition; there is at least 1 potty chair for every 5 potty-training infants. \302\267 Playgrounds are enclosed by appropriate fences and free of hazards. \302\267 Outdoor activity areas are supplied with age-and-size equipment in good condition. \302\267 Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin. \302\267 Food is stored appropriately and protected from contamination. \302\267 All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. \302\267 Menus shall be posted at least one week in advance in a place visible by the child\342\200\231s authorized representative, dated and kept on file for 30 days, and made available upon request. \302\267Uncontaminated drinking water shall be readily available both indoors and outdoors and provided by public water supply, bottles and sippy cups are made available if needed. \302\267 The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall. The facility utilizes: wood chips. \302\267 Sign in/Sign out record was reviewed and does not meet regulation requirements (See LIC809-D) \302\267 A Staff member is present with current Pediatric CPR/First Aid which expires on 05/10/2027. \302\267 Opening and closing staff member\342\200\231s CPR/First Aid expires on 05/10/2027. \302\267 Director completed Health and Safety Training \342\200\223 04/03/2016. \302\267 A review of children\342\200\231s records was conducted, and records were found to be incomplete during this inspection (See LIC809-D). \302\267 Disaster drills to be conducted every six months \342\200\223 last drill conducted on 01/09/2025 (See LIC809-D) \302\267 A review of staff records indicates that all staff present meet minimum qualifications for the position for which they were hired. \302\267 The Licensee was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 A review of staff records on 07/16/2025 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. \302\267 The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA discussed the safe sleep regulations with Licensee Tammy Murphy and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed Licensee Tammy Murphy of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee Tammy Murphy was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA Taityana Benson informed Licensee Tammy Murphy that this report dated 07/16/2025 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Taityana Benson informed License Tammy Murphy to provide a copy of this licensing report dated 07/16/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
