What the inspector wrote

On January 6, 2026, at 02:31pm, Licensing Program Analyst (LPA) Zirbes conducted a case management deficiencies inspection to address information received regarding Neverland Child Care Center. LPA met with Licensee, Chelsea Vinson. Present during today's inspection were six toddlers and 11 preschool children. Based on a review of the facility records and interviews, the Department discovered a reportable incident occurred on August 25, 2025. Per interviews, on August 25, 2025, after 03:00pm, the ceiling in the toddler classroom started leaking. For the safety of the children, the toddlers were removed from the classroom and relocated to the preschool classroom. Staff reported a bucket was placed under leak, and the landlord for the Center was contacted to report the leak. Interviewees reported that the toddlers were able to return to the classroom the next day. Facility records revealed the ceiling was repaired in October 2025. Per LPA review of the Departments records, this incident was not reported to the Department. Administrator confirmed the incident was not reported to the Department because the ceiling leaked one time and the children were not affected. Based on record review and interviews, the leak in the ceiling of the toddler classroom was an unusual incident that required the toddlers to be relocated to another classroom for their safety therefore this was a reportable incident. A type B citation for regulation 101212 (d)(1) (C) Reporting Requirements is being issued as this was a potential risk to the children in care. Refer to LIC 809D. Notice of Site Visit was given and must remain posted for 30 days. An exit interview was conducted with Licensee, Chelsea Vinson. Appeal rights and a copy of the report were provided to Licensee, Chelsea Vinson.

This requirement was not met as evidenced by: Per record review and interviews on August 25, 2025, the ceiling in the toddler classroom started leaking, which required the toddlers to be relocated for their safety, this unusual incident was not reported to the Department as required. This poses a potential Health, Safety, or Personal Rights risk to persons in care.
