Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

PERSONAL RIGHTS
The licensee shall ensure that each child is accorded the following personal rights:To be accorded dignity in his/her personal relationships with staff and other persons.This requirement was not met as evidenced by: Based on LPA interviews and record review Staff 1 yelled at Child 1 in front of other day care children. Which poses an immediate Health, Safety, or Personal Rghts risk to children in care.

The licensee shall ensure that each child is accorded the following personal rights:To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule...threat, mental abuse ... interference with functions of daily living... This requirement was not met as evidenced by: Based on LPA interviews and record review Staff 1 handled Child 1 with lack of patience and forced Child 1 into their seat while Child 1 was experiencing an episode of anxiety which poses as an immediate Health, Safety, and Personal Rights risk to the children in care.

Upon occurrence, during operation of child care center any events specified... a report shall be made to CCLD by telephone,fax ...during business hours...written report within 7 days ...Any unusual incident... threatens the physical, emotional health,safety of any child. This requirement was not met as evidenced by: Based on LPA interviews, observation and record reviews the facility Licensee and staff failed to report an unusual incident involving Staff 1 and Child 1 to CCLD, which poses a potential risk to the Health, Safety, and Personal Rights risk to children in care.
