Inspector’s narrative

What the inspector wrote

On 6/24/21 at 12:42 PM Licensing Program Analyst (LPA) Patricia Berry conducted a case management inspection. Licensee met with director and LPA was granted access into the facility. LPA toured facility and took census. LPA observed 12 children in the facility during the visit. On 6/14 Community Care Investigation Bureau (CCIB) was called regarding allegations that the facility is not being kept up. According to CCIB report there are several trees rotting and damaged, and that there are carpenter bees that are taking over. The report states the yard that the children in care play in is "filled with weeds, up to 3-4 feet tall". Code enforcement did an investigation on the outside of the facility and noticed a few problem areas which were dead grass and weeds. LPA did tour the playground and did not observe any carpenter bees, trees that were rotting or weeds that were 3-4 feet tall, however, the school-age playground is in disarray. LPA observed dead grass, dead leaves, weeds that were dead and dirty sand on the playground. Director stated the owner is cleaning and repairing the playground but it will not be ready for use until fall. Director stated the children are using the preschool playground only. LPA observed the preschool playground to be safe for children to play in and is clean. LPA informed director that pictures will need to be sent to Community Care Licensing before the school-age children can play in the school-age playground. LPA also informed director to add sand or another form of cushioning under the structures to be able to absorb a fall on the school-age playground. LPA interviewed school-age children who confirmed they do not play on the school-age playground at this time. Interviews confirmed children are not using the school-age playground, so at this time LPA determined there has been no violation of Title 22 regulations. Exit interview conducted with director and report given. LPA observed director post Notice of Site Visit.
