What the inspector wrote

On 06/29/21 at 3pm Licensing Program Analysts (LPAs) Justin Giese and Giselle Carbullido made an unannounced visit to the Facility for another purpose. LPA met with Facility Director, Shannon Garcia. During the visit, LPAs requested to review LIC9040, facility roster. Director confirmed the facility did not have a current or completed roster to review. See LIC9102 for technical advisory An exit interview was conducted with Director Shannon Garcia A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS A copy of this report was provided to the licensee on this date and must be made available to the public upon request for the next 3 years.

    LIMITATIONS ON CAPACITY

    101161 Limitations on Capacity: (a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license... This requirement is not met as evidenced by: Based on LPAs direct observations and staff interviews, the facility is providing care for school aged children and did not operate according to the conditions and limitations specified on the license. This poses an immediate health and safety risk to the children in care.

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229 Responsibility for Providing Care and Supervision. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... This requirement is not met as evidenced by: LPA's interviews with Director and staff confirm that on 06/09/2021 unqualified staff were left to supervise a classroom of children that were conducting their distance learning. This poses an immediate health and safety risk to children in care.
