Based on the observation & interview, the Licensee did not meet the above regulation which poses an immediate Health & Safety Rights risk to the children in care. During the facility tour of the school age program located in a detached bungalow, the LPA was unable to observe a working carbon monoxide detector in the . Staff stated the battery was dying which resulted in the carbon monoxide detector being unplugged and put in a cabinet.

OUTDOOR ACTIVITY SPACE:The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. This requirement was not met as evidenced by: LPA's observed the gate between the facility and the parking lot was unlocked. This is a potential hazard to the children in care.
