On date and time listed above, Licensing Program Analyst (LPA) Chase Atherton arrived at the facility to conduct a case management inspection. LPA and was granted entry by substitute Director Gladys Arreola and LPA informed them of the purpose of the visit. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 5/14/26. It indicates that the facility reviewed footage of an incident that occurred on 5/11/26, which showed that an inappropriate action occurred between two children while no staff members where supervising or observing both children. No injuries were reported. On the date of the incident, once facility staff became aware of this incident as it was occurring, they immediately separated the children and began supervising both children. Based on the information gathered, the following violations have been identified: the facility did not provide visual observation to all children at all times. See LIC809D for cited deficiencies of the California Code of Regulations, Title 22, Div. 12. LPA Chase Atherton informed Director Natasha Abuata that this report dated 5/20/2026 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Chase Atherton informed the Director Natasha Abuata to provide a copy of this licensing report dated 5/20/26 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Director Natasha Abuata and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to facility staff.
