On 03/22/2023 ) Licensing Program Analyst (LPA ) Susan Brewer and Licensing Program Manager (LPM) Kimberly Williams, arrived to the facility to conduct a case management visit, in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 03/13/2023, regarding possible inappropriate contact between 2 children. Present was Director Kim Smith, who stated that the facility is on spring break from 03/20/23 - 03/24/23. A census was taken of 0 children in care. Facility records were reviewed for subject children and additional interviews will be needed. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided to the licensee. An exit interview was conducted and a copy of this report was provided to Director Kim Smith.
