On 04/11/2023 Licensing Program Analyst (LPA) Susan Brewer arrived to the facility to conduct a case management follow-up visit, in response to the receipt of an unusual incident report (UIR) from the facility. An Initial visit was conducted on 03/22/2023, while the facility is on spring break from 03/20/23 - 03/24/23. The UIR was received by the licensing agency on 03/13/2023, regarding possible inappropriate contact between 2 children. LPA met with Director Kim Smith and a census was taken of children in care. On today's visit, LPA S.Brewer, conducted interviews, reviewed and gathered facility records. Additional information and interviews will be needed with pertinent parties not present during the visit. Upon completion of the review, the outcome and/or recommendations will be provided to the licensee. An exit interview was conducted and a copy of this report was provided to Director Kim Smith.
