What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a Case Management investigation, regarding a self-reported incident. The Department received a self reported Unusual Incident Report regarding Child #1. It was reported that Child #1 approached their teacher and stated that their arm was hurting. Documentation obtained from the facility indicated that the child had been playing on the playground, tripped over their shoes, and fell, injuring their wrist. The facility reported that an ice pack was applied and the child’s parent was notified immediately. It was further reported that the parent took the child to urgent care for further evaluation. It was also reported that Child #1 had reportedly sustained an injury over the previous weekend outside of the facility and that the fall at the facility may have re-aggravated the prior injury. It was reported that Child #1 returned to the facility the following day and resumed normal activities. As of today, Child #1 is reported to be fine and back to normal. LPA investigated the reported incident by conducting interviews, collecting documentation, and making observations. It was determined that the facility took appropriate measures to respond to the injury and properly notified the parent and the Licensing Department. Based on information obtained during today’s visit, no violations of Title 22 regulations were identified. No deficiencies were cited during today’s visit. LPA conducted an exit interview with Director Kim Smith, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
