Inspector’s narrative

What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a Case Management investigation, regarding a self-reported incident. The Department received a report regarding an incident involving Child #1. It was reported that Child #1 informed their teacher that another child (Child #2) had pulled their arm. It was stated that Child #1's teacher assessed Child #1’s arm and gently moved it in different directions. It was reported that Child #2 was able to move their arm and returned back to play time without apparent discomfort. However, it was then reported hat later in the day, Child #1's teacher observed that Child #1 was not using the arm that was previously looked at. It was stated that Child #1 was favoring their other arm, which was not usual. It was then reported that Child #1's parent was contacted and that that Child #1 was taken to urgent care that same day. It was reported that no X-Rays were required, per the doctor, and that Child #1’s condition improved quickly. The facility reported that Child #1 returned to the facility within two days of the incident and resumed normal activities. It was stated that Child #1 is now back to normal. LPA investigated the reported incident by conducting interviews, collecting documentation, and making observations. It was determined that the facility responded appropriately, monitored Child #1, and notified the parent and the Licensing Department as required. Based on information obtained during today’s visit, no violations of Title 22 regulations were identified. No deficiencies were cited during today’s visit. LPA conducted an exit interview with Director Kim Smith, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
