Inspector’s narrative

What the inspector wrote

On date and time listed above, Licensing Program Analyst (LPA) Raymond Moorehead arrived at the facility to conduct a Case Management investigation, regarding a self-reported incident. The Department received a report regarding an incident involving Child #1. It was reported that Child #1 was standing in line transitioning from recess back to the classroom when Child #1 tripped over their own feet and fell, resulting in a bump to the forehead. Staff interviews disclosed that Child #1 was carrying their water bottle, coat, and lunch pale at the time of the reported fall. The facility reported that swelling appeared on the left side of Child #1's forehead and that ice was applied immediately. It was stated that Child #1 was picked up by their parent and taken to urgent care for evaluation. It was reported that the swelling had significantly decreased between the time of the fall and the time of pick up of Child #1. It was stated that Child #1 has returned to the facility the following day and was reported to be back to normal. During today’s visit, LPA conducted interviews with pertinent individuals and observed the area where the fall occurred. No hazards were observed, and the incident appeared to be accidental. LPA investigated the reported incident by conducting interviews, collecting documentation, and making observations. It was determined that the facility responded appropriately and notified the parent and the Licensing Department as required. Based on information obtained during today’s visit, no violations of Title 22 regulations were identified. No deficiencies were cited during today’s visit. LPA conducted an exit interview with Director Kim Smith, and provided a copy of this report. A Notice of Site Visit was issued and must remain posted for the next 30 days.
