(1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation.This requirement is not met as evidenced by: Based on records review, interview, and self-report from the facility, the facility did not maintain supervision or visual observation of 4 infants, for about 2 to 3 minutes, while infants were on the facility playground. This poses/posed an immediate health, safety or personal rights risk to persons in care.
