What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. LPA took a census at the start of this visit. The UIR was received by the licensing agency on 2/23/2026. It indicates that on 2/23/2026 4 infants were left outside on the playground for about 2 to 3 minutes, without any staff supervision or visual observation, while all staff were inside. Staff then realized the infants were outside by themselves and brought all 4 infants back inside and resumed supervision and visual observation. No injuries were reported. Facility records were reviewed and pertinent parties were interviewed. Based on the information gathered, the following violations have been identified: 4 infants in care were left without supervision and visual observation for about 2 to 3 minutes. See LIC809D for cited deficiencies. A civil penalty has been issued in the amount of $500. If a Civil Penalty has been assessed during this inspection, payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH. LPA Chase Atherton informed Director Margaret Van Duzer that this report dated 3/3/2026 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. SEE LIC809C for a continuation of this report... Also, LPA Chase Atherton informed the Director Margaret Van Duzer to provide a copy of this licensing report dated 3/3/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given to Director Margaret Van Duzer and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to the Director Margaret Van Duzer.
