REPORTING REQUIREMENTS
Based on the observation/interview/record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arrival the LPA was greeted by S2 who introduced themself as the Director. The LPA was informed the Director on file has not been the Director for about a month. LPA observed an LIC 610 Emergency Disaster Plan For Child Care Centers dated 7-25-2025 which lists S1 as the Director.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the staff file review the LPA was unable to review a Preventative Health & Safety certificate for at least one staff present.

Based on the record review, the Licensee did not meet the above regulation which poses a potential health, risk to the children in care. During the children’s file review the LPA was unable to review an LIC 701 Physician’s Report for C1.

Based on the observation, the Licensee did not meet the above regulation which poses a potential health, risk to the children in care. During the facility tour the LPA was unable to inspect at least one carbon monoxide detector in the building. During the churches remodel the plug-in detectors were removed and never put back. Director was informed the smoke detectors were combo, however, it was verified they were not.
