Inspector’s narrative

What the inspector wrote

****THS IS AN AMENDED REPORT. ORIGINAL DATED 06/12/2025**** It was alleged a child was told to face the corner of a wall during time out, which is described as a form of "punitive control." ." During interviews with relevant parties, it was confirmed children are placed in time out when they are not following directions; however, children do not face the corner of a wall. A second allegation involved an uncleared adult providing care and supervision to the children. Upon conducting interviews and reviewing documentation, the LPA was unable to determine whether a volunteer who assisted with the children was a minor at the time of their involvement with the day-care or had already turned 18-years-old, which would have required the volunteer to have a criminal record clearance. Due to conflicting information obtained from what was alleged, the evidence collected was not sufficient to substantiate nor refute the above allegations, therefore the allegation is UNSUBSTANTIATED. A finding of unsubstantiated means although the allegation may have happened, or could be valid, there is not a preponderance of evidence to prove the allegation occurred. An exit interview was conducted with the licensees, Ebony Green, Essence Merriwether. Appeal Rights were discussed and issued, a copy of this report was provided, and a Notice of Site (NOS) Visit was issued. The Notice of Site Visit (LIC9213) shall be posted where the parent/guardian of children enter and exit the facility and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. A copy of this report must be made available for the next three years.
