What the inspector wrote

On 4/18/2025 at 9:25am, Licensing Program Analysst (LPAs) Chase Atherton & Aman Lama arrived at the facility to conduct a Case Management-Legal inspection regarding the Decision and Order dated 01/31/2025 and effective 04/07/2025. Xuqing Zhuang stated that they can translate the following report for Director Feng Xue and did so for the entire report. The Decision and Order is for the exclusion of an individual, Simei Niu, and excludes this individual for the remainder of her life from the effective date of this decision. The following was observed and/or discussed: The facility representative Xuqing Zhuang stated that the individual has not been working in the facility since January or February 2024. LPAs toured the facility inside and out to ascertain whether or not, Simei Niu was present in the facility. The LPAs did not observe Simei Niu, at the facility during the inspection. The facility representative Xuqing Zhuang stated that Simei Niu WAS NOT present in the facility on today’s date and HAS NOT been at the facility since January or February 2024. LPA discussed the Decision and Order with facility representative Xuqing Zhuang and Director Feng Xue. Feng Xue's signature below confirms that they received a copy of the Decision and Order on 04/18/2025, effective 04/07/2025. Director Feng Xue understands that individual, Simei Niu is excluded from 04/07/2025 to the end of their life, and that the facility is to remain in full compliance with Tittle 22 Regulations. LPAs informed Director Feng Xue that the department may conduct a follow-up site inspection, to ensure compliance with the Decision and Order. An exit interview was conducted, and a copy of this report was provided to Director Feng Xue who understands a copy of this report must be made available to the public for three years. A notice of site visit was given to Director Feng Xue and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
