Follow-up on corrections·May 16, 2025

On 5/16/25 LPA conducted a case management in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 5/13/25. It indicates that a child was left outside on the playground, alone. Facility staff was interviewed. Facility self reported the incident to Community Care Licensing immediately and informed the parent. Based on the information gathered, the following violation has been identified: 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. See LIC809D for cited deficiency of the California Code of Regulations, Title 22, Div. 12. LPA informed director, that Type A citation must be reported to all authorized representatives/guardians of all children currently enrolled by the next business day, or the next day children are in care, and all newly enrolled children for the next 12 months from the date of citation. The signed Acknowledgement of Receipt LIC 9224, must be placed in child’s file for verification. An exit interview was conducted with director. During the exit interview, appeal rights were discussed/provided, Notice of Site form and LIC 9224 Acknowledgment of Receipt, and a copy of this report was provided. Civil Penalty LIC421 IM assessed today for $500.00 and provided to the director. Notice of Site visit must be posted for 30 days.
