Inspector’s narrative

What the inspector wrote

Pertinent parties stated two teachers noticed the child outside the classroom and brought the child back inside. Pertinent parties stated after reviewing video footage, it was determined the child was in the hallway for approximately 12 seconds. Based on interviews conducted, a child was left alone in the hallway, absence of supervision; therefore, the preponderance of evidence standard has been met, and the allegation is substantiated. See LIC 9099D for deficiency. Civil Penalty assessed today for $500.00 due to absence of supervision. Allegation: Facility operated out of ratio It was alleged the facility was operating out of ratio. LPA interviewed all pertinent parties, including staff. Pertinent parties stated on occasion, classrooms do operate out of ratio. Pertinent parties stated once the classrooms are out of ratio, it could take up to 30 minutes for additional staff to arrive. Based on interviews conducted, the facility does operate out of ratio; therefore, the preponderance of evidence standard has been met, and the allegation is substantiated. See LIC 9099D for deficiency. Exit interview conducted with director, appeal rights, acknowledgement of receipt and Notice of Site Visit issued. Notice of Site Visit must be posted for 30 days.
