What the inspector wrote

On this date and time, Licensing Program Analyst (LPA) Patricia Berry arrived at the facility unannounced for the purpose of conducting a Case Management \342\200\223 Legal/non-compliance inspection regarding a Decision and Order (D&O), which was ordered on November 22, 2024, and has become effective beginning December 2nd, 2024. The Decision and Order is regarding the exclusion of individual, Adult #1, who was once employed at the facility. The purpose of the inspection was explained to Facility Director, Ashlee Stuehrmann. LPA toured the facility and took a census. The subject, Adult #1 was not observed at the facility and their last date of employment with this facility was May 23, 2024. A copy of the Decision and Order was mailed to the Respondent and facility, An additional copy was provided to Facility Director, during today\342\200\231s inspection. Facility Director acknowledged receipt and understanding of the Decision and Order which reads: \342\200\234Respondent\342\200\231s appeal of complainant\342\200\231s request to exclude her from licensed facilities is denied. Respondent is prohibited from employment in, presence in, and contact with clients of, any facility licensed by the Department or certified by a licensed foster family agency, or any resource family home, and from holding the position of member of the board of directors, executive director, or officer of any licensee of any facility licensed by the Department, for the remainder of Respondent\342\200\231s life, until respondent successfully petitions for reinstatement pursuant to Government Code section 11522.\342\200\235 No deficiencies cited during today\342\200\231s inspection. An exit interview was conducted, appeal rights discussed, and a copy of this report and LIC 811 confidential names list was provided to Facility Director, Ashley Stuerhmann. A notice of site visit was issued and must remain posted in a prominent location within the facility for a period of 30 consecutive days.
