What the inspector wrote

On March 20, 2024, Licensing Program Analyst (LPA) Taityana Benson arrived at the facility to conclude the investigation in a separate matter, a previous inspection was conducted on October 31, 2023. LPA met with Licensee, Shauntel Joyce and conducted a tour of the facility inside and outside. Per interviews conducted, and information gathered, the investigation revealed Licensee, Shauntel Joyce observed bruising on a daycare child on more than one occasion during drop off in the month of October 2023. The licensee communicated their concerns with the daycare child parent and received reasonable accounts of how the injuries occurred. The license did not believe the daycare child required medical attention at the time but later had concerns of the explanation the parent provided in regard to how the injury occurred. It was discovered during record review, t he licensee did not notify the department via telephone or fax in regard to the incidents the next business day and during normal working hours. Furthermore, the licensee did not inform their assigned Licensing Program Analyst of the incident within 24 hours and did not submit an Unusual Incident/Injury Report (LIC624) to the department within seven days following the occurrences of the incidents. LPA informed the Licensee that in addition to notifying the Licensing Agency, as a mandated reporter she is required to report all suspected child abuse inquiries to the local Child Protective Services. SEE LIC809-D for deficiency cited A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and report was reviewed with Licensee, Shauntel Joyce .

Based interviews on record review, the Licensee did not comply with the section cited above, Licensee did not notify the department of unusual incidents within 24 hours when suspicion of child abuse was present of a daycare child with seven days, which poses a potential risk to the Personal Rights of persons in care.
