On 10/31/25 at 8:25 am, Licensing Program Analyst (LPA) Patricia Berry conducted a subsequent case management visit in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 10/20/25. It was alleged that a child said inappropriate things to another child. Children and staff were interviewed. Pertinent parties stated they had no idea about the incident. Pertinent parties stated the two children never played together. LPA observed a video of the two children playing in the sandbox and LPA observed the two children playing away from each other. Based on information gathered and LPA's own observation of review of a video, the facility acted appropriately, and no violations have been identified. The facility conducted their own internal investigation. Facility informed Community Care Licensing of the incident immediately and facility informed authorized representatives involved. An exit interview was conducted with director, and a copy of this report was provided to facility staff along with appeal rights and notice of site visit. Notice of Site Visit must be posted for 30 days.
