What the inspector wrote

Licensing Program Analyst (LPA) Aman Lama arrived at the facility to conduct a Case Management (CM) for the purpose of clearing citations, aka a POC visit. A previous POC visit was made on 02/09/2026, for the same corrections. Upon arrival, LPA was met with facility representative, Alma Garcia. Due to the site director not being available for a tour, LPA toured the indoor and outdoor areas of center by herself. LPA was later met with the facility representative, Miriam Garcia. On 09/18/2025, and 02/09/2026, LPA made unannounced visits to facility for other purposes. During todays visit, it was disclosed that there are some pending POC's that have not been corrected as of yet. The center was issued repeat type B citations. These deficiencies have been outstanding without correction. Therefore, due to missed POC dates and failure to correct, the deficiencies will be cited again during today's visit. Furthermore, LPA observed and cited new Type A and Type B deficiencies for being out of compliance with Title 22 Regulations. The facility was found be in violation of the following regulations: Repeat citations: CCR 101216(g)(2) , CCR 101220(a) CCR 101238(g)(1) -SEE LIC809D for further information. New citations: 101215.1(b), 101162(a)(1), and - SEE LIC809D for further information. This report has repeat violations, and a civil penalty of $250 each will be assessed. 2 Civil Penalties have been assessed during this inspection. Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH . See LIC809-D for cited deficiencies. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Miriam Garcia. Also, LPA Aman Lama informed the facility representative, Miriam Garcia to provide a copy of this licensing report dated April 30 , 2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled, or newly enrolled by the next business day or the next day the child(ren) is(are) in care. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification and kept on file for 12 months from the date of this report. LPA Aman Lama informed facility representative, Miriam Garcia that this report dated April 30, 2026 document(s) (1) Type A citation(s) which shall be posted for 30 consecutive days as there is(are) immediate risk(s) to the health, safety, or personal rights of children in care. An LIC9224 was provided.

    (a) No person or legal entity shall advertise or represent itself as a licensed child care center without first obtaining a current valid license from the Department.(1) Licensees shall reveal each child care center license number in all advertisements in accordance with Health & Safety Code Section 1596.861. This was not met as evidenced by: LPA observed a large banner at the front of the center, advertising care. However, no facility number was observed to be on the banner. This poses a potential health and safety risk to persons in care.

  • CHILD CARE CENTER DIRECTOR QUALIFICATIONS AND DUTIES

    (b) All child care centers shall have a director. This was not met as evidenced by: The previous individual appointed as the site director did not qualify. Currently, there is no site director appointed to this site. This poses a potential risk to he health/safety to children in care.

  • PERSONNEL REQUIREMENTS

    (g) All personnel, including the licensee, administrator & volunteers, shall be in good health and be physically& mentally capable of performing assigned tasks. (2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening... On 02/09/26, based on a 2nd record review for S1 and S4, S4 was still missing health screening. LPA was told the appointment was scheduled, however no correction was submitted. This poses a potential health/safety risk to persons in care

    CHILD'S MEDICAL ASSESSMENTS

    (a) Prior to/ w/in 30 calendar days following enrollment of a child, licensee shall obtain a written medical assessment of child. This enables licensee to assess whether the center can provide necessary health related services to the child. A 3rd review of C5's file still does not have the above on file.

    (g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. (1) Storage areas for poisons shall be locked. This was not met as evidenced by: During the tour, LPA observed 2 gates to a hallway between one preschool classroom to the next, on the side of the kitchen area accessible to children. There were two gates still standing, however, they were not latched on, as the latches were broken. The area contains paint enamel and paint primer cans, which are items that could pose a danger to children,
