Licensing Program Analyst (LPA) Aman Lama arrived at the facility to conduct a Case Management (CM) for the purpose of clearing citations, aka a POC visit. Upon arrival, LPA was met with facility representative, Miriam Garcia. Due to the site director not being available for a tour, LPA toured the indoor and outdoor areas of center by herself. LPA was later met with the owner, Fernando Chavez. On 09/18/2025, LPA made unannounced visit to facility for another purpose. During the 09/18/2025 visit, LPA reviewed pertinent facility documentation. The center was issued type A and B deficiencies for being out of compliance with Title 22 Regulations. Facility representative was unable to provide Plan of Corrections (POCs) for all of the citations. These deficiencies have been outstanding without correction. Therefore, due to missed POC dates and failure to correct, the deficiencies will be cited again during today's visit. The facility was found be in violation of the following regulations: CCR 101202(b), HSC 1596.7995(a)(1) -SEE LIC809D for further information. These are repeat violations, and a civil penalty of $250 will be assessed for staff records. 2 Civil Penalties have been assessed during this inspection.
