Inspector’s narrative

What the inspector wrote

A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility regarding a sister location across the street, Montessori School of Chino Hills 364841673. The UIR was received by the licensing agency on 5/16/2025. It indicates due to a car running into and breaking one wall of one room of the sister location (364841673), the the children that normally used that room were temporarily relocated to this facility (364846333). On the date and time listed above Licensing Program Analysts (LPAs) Chase Atherton and Elyse Jones arrived and the facility and informed the facility representative of the purpose of this visit. LPAs toured the facility and took a census. Facility records were reviewed, and pertinent parties were interviewed. Based on information gathered, the facility acted appropriately, and no violations have been identified. The facility notified licensing per Title 22 regulations. The room the facility used to care for the displaced children had an appropriate fire clearance per Title 22 regulations. The facility also requested a waiver for the the school age room at this facility to be used for the displaced preschool age children. LPAs informed the facility representative that all Title 22 regulations must be followed until a waiver is approved and granted. LPAs observed that at this time all preschool age children have returned to their original facility and the school age room is no longer being used for the displaced preschool age children. Therefore, at this time, a waiver is not needed as all regulations regarding the usage of the school age room are currently being followed. A different LIC809 report dated 5/22/2025 addressed the sister facility's (364841673) car accident. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed w ith Facility Representative Ranjanie Serasinghe .
