On date and time listed, Licensing Program Analysts (LPAs) Taityana Benson and Aman Sharma arrived at the facility to conduct a pre-licensing inspection for the purpose of a relocation. The Applicant was previously licensed and has relocated to a new facility. Present during this inspection were: Alma Chavez. LPA toured the facility, inside and out and the following was observed and/or discussed: Normal days and hours of operation are: Monday through Sunday, 23.5 hours per day OFF-LIMIT AREAS INCLUDE: 2 nd Floor, Laundry Room, and Garage. \302\267 Fire Clearance Granted 12/12/2023 \302\267 Appropriate fire extinguisher, smoke detector and carbon monoxide detector are present and were tested by the applicant during this inspection. Fire extinguisher, smoke detector and carbon monoxide detector are in working order. \302\267 All hazardous items are inaccessible \302\267 Toxins locked \302\267 No guns or weapons present as of this date per Applicant, Alma Chavez. Applicant understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations. \302\267 Stairs are barricaded (Wrought Iron Gate) \302\267 Fireplace is properly screened to prevent access by children \302\267 Storage of poisons is inaccessible to children and locked \302\267 Verification of control of property on file (Grant Deed) \302\267 Facility Sketch and Emergency Disaster Plan are posted (2 nd Floor Facility Sketch Missing) \302\267 Mandated Reporter Training completed on 01/19/2023 \302\267 Pediatric CPR and First Aid Card - expire 02/2025 \302\267 Health & Safety Certificate - completed on file \302\267 Applicant understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position when not in use. \302\267 Clean, safe and age appropriate toys \302\267 There are no toxic plants observed at this time \302\267 The Applicant was informed of their reporting requirements and is provided with the Regional Office\342\200\231s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov \302\267 Resident and/or staff records reviewed on 12/20/2023 indicate that all adults who require caregiver background checks have received all required clearances or exemptions. \302\267 The Applicant can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov The following was discussed with the applicant(s): - Pre-Licensing Visit Packet provided - Failure to meet the posting requirements shall result in an immediate $100 civil penalty. - Documentation of fire & earthquake drills to be conducted every six months - Responsibilities of being a mandated reporter - Responsibility to know the regulations for anyone providing care - Inaccessibility of hazards must be constantly reassessed depending on the children in care - Current facility\342\200\231s phone numbers must always be on file with the licensing office - Baby walkers, baby-bouncer, and walker-jumpers are prohibited (16 CFR 1500.86(a)(4)) LPA reviewed with Applicant, Alma Chavez the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. LPA discussed the safe sleep regulations with Applicant, Alma Chavez and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Applicant, Alma Chavez of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information , see PIN 22-02-CCP . When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Before licensure, the following needs to be corrected/completed: 1. Fire Extinguisher (Re-Charge) 2. Facility Sketch (2 nd Floor) 3. Backyard Rear Playground Area: Install cushion underneath high climbing area to absorb children falls. Once all corrections have been verified, the application for a Large Family Child Care Home will be submitted for approval with a maximum capacity (when there is an Assistant present): 12 \342\200\223 No more than 4 infants. Capacity 14 \342\200\223 no more than 3 infants, 1 Child in kindergarten or elementary school and 1 child at least age 6. The Applicant , Alma Chavez, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.
