What the inspector wrote

On the above noted date and time, Licensing Program Analyst's (LPA's), Diana Brasel and Griselda Castellon conducted an announced Pre-Licensing inspection for a Change of Ownership. Upon arrival, LPA met with Applicant Jianxia (Emma) Liu. Applicant is requesting to be licensed for 72 children, 48 preschoolers 3 through 5 years of age in preschool # 1, 2, and 3 and 24 toddler option children ages 18 - 36 months of age in toddler classroom. During today's visit the facility has requested to submit an updated LIC 200A to reflect 60 preschool children and 12 toddler option children. The applicant provided a translator for the visit. Hours of operation will be Monday - Friday 6:30 am - 5:00 pm. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Jianxia (Emma) Liu and Director Trina Hohenshelt staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, director stated that they will be in stored in the office. A fully equipped first aid kit is located in kitchen. There is an operational carbon monoxide detector on site located in each room. All required licensing documents were observed posted in the office. Children will be signed in and out at the office area. LPA's continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for toddler option is 587.77, which is sufficient to accommodate the new requested capacity of 12 children, total indoor activity space measured for preschool is 3781.1, which is sufficient to accommodate the requested new capacity 60 children. LPA's observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via drinking fountains. In the toddler option room filtered water in a jug with disposable cups/children's water bottles will be available. Fire clearance was granted on 10/24/2024. An updated fire clearance will be requested to reflect the change in the capacity for both toddler option and preschool. LPA's observed 1 sink and 1 potty chair to be available for the toddler option, 10 sinks and 6 toilets are available for the preschool children's use. These are sufficient to accommodate the new requested capacity of 12 toddler option children and 60 preschool children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the office. Facility will provide am and pm snacks only, children's lunches will be provided by the parents. The kitchen area currently includes a refrigerator, microwave, toaster ovens and 3 sinks. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a locked cabinet. The facility currently has a fully fenced playground area. Fencing is brink wall and wrought iron. On the toddler option playground there a areas that are under 4 feet tall (see correction needed) The total square footage for all the outdoor activity space for the toddler option is 2140.47, which is sufficient for the number being requested, total square footage for the preschool is 6557.00, which is sufficient to accommodate the requested capacity. Shade is provided via permanent canopies and wind sail. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is an age appropriate climbing structure for both programs on each playground which is properly anchored. There is adequate cushioning in fall zones of climber provided by wood chips on the toddler option playground and rubber mulch on the preschool playground. Drinking water is available via drinking fountains. LPA observed all hazardous items on the playground to be inaccessible to children. (there will be corrections needed to both the outdoor playgrounds) Jianxia (Emma) Liu was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Sides of the changing table does not have a 3\342\200\235 high lip (see correction needed), the changing table is within arm\342\200\231s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Criminal Record Clearance - Child Care Centers: Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. This facility plans to provide Incidental Medical Services \342\200\223 IMS. For IMS information, see PIN 22-02-CCP . A Plan of Operation that includes IMS must be submitted to the Department. MyChildCarePlan.org--Child Care Centers and Family Child Care Home: Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Subscribe to CCLD important information - Child Care Centers and Family Child Care Homes: Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA's discussed with the applicant that the facility needs to refer to both the toddler option and the infant regulations. The following corrections are needed prior to the issuance of the license: 1. Preschool room 1: a. Proof that the children's water fountain located at end of room has been repaired. b. Proof that the cabinet door has been repaired to prevent access to compressed wood. c. Proof that a latch has been installed on the reptile aquarium stand. d. Proof that the door leading to preschool room 3 has been cleaned and repaired. 2. Preschool room 2: a. Proof that the staff restroom door frame has been repaired and door made inaccessible. b. Proof that the cabinet doors on the sinks have been repaired to prevent access to compressed wood. c. Proof that the one water damage ceiling tile has been replaced. d. Proof that the reptile aquarium has had a covering added to the top of the aquarium and a latch on the door portion of the aquarium. e. Proof that the gate leading the off limits room has been replaced. f. Proof that the ledge with peeling/chipped paint has been repaired. 3. Preschool room 3: a. Proof that the cabinet door has been repaired to prevent access to compressed wood. -Corrections continued on LIC 809C: b. Proof that all dark wood shelves have been anchored. c. Proof that the reptile aquarium has had a covering added to the top of the aquarium and a latch on the door portion of the aquarium. d. Proof that a 3" high lip has been added to the changing table, which the facility has stated will be used. 4. Toddler room: a. Proof of removal of refrigerator and microwave on top of refrigerator. b. Proof of steps/step stool has been added to children's sink to ensure access to the sink has been installed. 5. Preschool outdoor eating area: a. Proof that the wrought iron fence/gate has been repaired or replace, also repair/replacement to the area that has chipping concrete. 6. Preschool playground: a. Proof that the large shed has been either repaired or replaced to include wooden base. b. Proof that the large opening under the gate next to the large shed has been filled in to prevent access to the off limits area. c. Proof that all areas of chipped stucco on the building structure has been repaired. d. Proof that the small canopy polls have been repaired to prevent access to rust. e. Proof that the large canopy covering has been repaired or replaced. d. Proof that the stairs and rails on the climbing structure has been repaired. e. Proof that the long rusted metal rod across the top of the climbing structure has been repaired or replaced. 7. Toddler option playground: a. Proof that the canopy covering has been repaired or replaced. b. Proof that all areas of the wrought iron fencing with rust has been repaired. c. Proof that all areas of the wrought iron fencing that measures shorter than 4 feet tall shall be made to measure 4 feet tall. -Corrections continued on LIC 809C: d. Proof that all railroad cross ties beams have been replaced or repaired. 8. Proof that the facility has one tight fitting trash can for food waste has been installed. Applicant Jianxia (Emma) Liu understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied. LPA Brasel read report to applicant, applicant used translator app. Exit interview conducted and report was reviewed with the applicant, Jianxia (Emma) Liu.
