What the inspector wrote

On this date and time, Licensing Program Analysts (LPA) Laura Mejorado arrived at the facility to conduct a case management visit in response to the receipt of a self-reported unusual incident report (UIR). The UIR was received by the Riverside Child Care Regional office, on 2/18/26. The UIR documented an incident of a subject child sustaining an injury while in care. Upon arrival, LPA met with Director Nicole Ramirez and stated the purpose of the visit. Records were reviewed, video footage was reviewed and interviews were conducted. On 2/17/26, a child was running to wash their hands when one of their shoes slipped off causing them to trip and fall hitting their upper lip on the corner of the hand-washing sink. While reviewing the video footage the teacher was observed near the sink and witnessed the child fall. The teacher immediately attended to the child applying a wet paper towel to the injury. Another staff member came to assist and brought the child an ice pack. The facility then contacted the child\342\200\231s authorized representative and informed them of the incident requesting the child be picked up. Additionally, the facility took a picture of the injury and wrote an incident report that was sent to the authorized representatives via the facility app. LPA reviewed video footage of the incident which corroborated the information submitted to the department. The facility took appropriate action by completing self-reporting requirements as required for Unusual Incident Reports. Based on the information gathered and compiled during this visit no citations were issued, at this time. Exit interview conducted and report was reviewed with Director Nicole Ramirez. A notice of site visit was given and must remain posted for 30 days. A copy of this report must be made available to the public, at the facility site, for 3 years.
